We are providers for most insurance plans. If your plan requires a referral from your Primary Care Physician (PCP) please see that a referral has been issued prior to your visit.  The plans that we are aware of that require prior authorization have been marked with asterisks below, although the list may not be exhaustive.  Please remember to also bring your insurance identification card(s) and any claim forms provided by your insurance carrier. It is also important that you notify our office if your insurance carrier requires authorization or pre-certification prior to treatment being rendered and to avoid any reduction in your benefits. In these cases, we will review your insurance coverage with you. As part of our services we will file your claims at no charge. We do however, ask that if your insurance coverage changes for any reason, that you advise our office of the change as soon as possible.

All fees incurred in our clinics are due and payable at the time services are rendered, unless prior arrangements have been made. Our fees are within customary ranges and reflect the services and care you receive from our physician. You will receive a monthly statement which shows an itemization of the charges incurred and any payments made by you or your insurance carrier. Please also review the explanation of benefits you receive from your insurance carrier regarding any annual deductibles/copays your carrier has assigned as your responsibility as well.
