Your First Visit with Barbara J. Aung
It is our policy not to "double" book or otherwise overbook our schedule, so that we can devote the proper time to your visit. Therefore, Twenty four hour notice is required to cancel your appointment; otherwise there is a missed appointment fee depending on your circumstances. Should you need to cancel or change your appointment our office staff can be contacted during regular office hours and after hours you can either leave a message on our clinic answering machine or if you are familiar with out online scheduling system send an email that our staff will review on the next business day.
For your first visit with Dr. Aung office please take the following with you and arrive 15 minutes prior to your scheduled appointment time:
- insurance cards which will be copied
- any insurance required referral forms or document (you should contact your insurance prior to the visit if you have any questions about coverage.
- a list of all medications (including generic and non prescription products) currently used, and dosages
- any old records pertaining to your feet or your current problem
- if possible, a copy of your more recent general health summary or office visit from your primary physician such as a yearly history and physical. You also have the options of providing this information through our online appointment scheduling system instruction on how to utilize this system are located on at this link.
- you can also complete your patient registration paperwork in advance of your appointment by again utilizing our online system located here.
- shoes that you often wear (bring several pair) including work, casual, sport etc (old and worn out are fine)
- prior x-rays, MRI or other imaging tests and reports
- do not hesitate to write down in advance any questions you have so that you do not forget what you want to review at your visit. It is also helpful, if you have a considerably involved history, to provide a written summary that we can attach to your record. Remember, especially for more complex issues, or if you have been to several other offices, the small details may make the difference in guiding us to effective solutions
- check, credit card, or cash if you have insurance co-pay (due at time of visit)
Please note: plan on spending between 40 minutes to an hour at the office for your first New Patient visit. On a rare occasion there are sometimes urgent care visits that put the schedule slightly behind, we ask you be considerate of those instances, since that same extra time may be needed for you one day.

In order to serve you better and faster, we offer two ways to complete your new patient information in advance of your appointment.
Option 1: Complete the New Patient Process Online
We offer you the convenience of completing your new patient information online using a secure server. This system will also allow you to make confirmed appointments, reschedule, or cancel future appointments.
Option 2: Printable Form
If you have a printer, you can print out our new patient and medical history forms. After printed, bring the completed forms to our office on your initial visit.
